a boutsohead is derived from greek and english and can be used to describe anything or anyone in anyway pleased. In particular used in the context of insulting one for being an imcompetent fool. literally means one that has genitalia for brains. (hence the flexibility as this can be percieved as not only negative but also beneficial).
