Someone who received their MD from some "institution" in the Caribbean. Either a) someone who sucks so bad (MCAT less than 21), or b) an al qaeda sleeper agent, or c)some chick that couldn't even sleep her way into a US school or d) all of the above.
Captain Jack Sparrow wouldn't get checked for STDs from a Carribbean MD because they would do most of the testing online.
