a woman or man whos nipples are very similar is shape, texture and appearance to door nobs. ie. they are smallish at the base and then get a lot larger twards the end of the nipple. they appear swollen, as if people have tried to open something with them, like a knob.
"woaaah, check out the knob nipples on her!"
"she has door knob nipples"
"those nipples look like they could open doors"
