When same sex marriage became legal in the state of Iowa, this term became widely used to offend people.
Instead of using the normal version of "You're gay" and "Eat a dick faggot" this term adds sophistication to the insult.
This insult is generally only used in states where same sex marriage is legal.
"Hey man what are you doing?"
"I'm just fixing my hair real quick"
