The term was used to refer to a black female maid who not only cared for things around the house but was responsible for raising the children of the white family she worked for
A term derived from the old south after slavery, but during the time when it was still common for white people to have many black people working for them in lower end jobs. The term was not usually meant in a demeaning way either. Most families were quite attached to their "nigger nanny"

person 1: Oh dear I just don't know how you juggle all your housework and your children
person 2: Oh honey it's easy. I just love my nigger nanny. She's a big help with the kids!
