The name given to a new sexual act (to some, fetish) in which someone (from Cincinnati) asks their sexual partner who isn't from Cincinnati if they have ever been to Over the Rhine. The partner who answers "no" is shown what it is like when the other person pulls down their pants, puts their rear in the person's face and farts blood all over them. "That's what it's like!"
