What is Serial Fuckup?

Somebody, who irrespective of being "highly intelligent" keeps on making the same mistakes and causing problems for other people through those mistakes. Can also be termed a dumb shit or just useless. The iq level of this person is sometimes as high as a garden slug's but never higher.
"Can you believe those suppliers, it the tenth time they messed up my order, what a bunch or serial fuckups"
