When you have made a mess of your work, without being aware of it. This is generally through no fault of your own, due to the fact that you are in over your head and are oblivious to how your job should actually be done.
You assign tasks to be done on a project before the tasks could possibly be accomplished causing cost over-runs. Your boss realizes that you have just S.Y.'d it.
